Charter Waiver & Media Release
Please review the waiver and media release terms below, then sign it digitally online before your charter departs.
Participant Details
Your full name, date of birth, phone, email and emergency contact are collected when you sign the waiver digitally before your trip.
Activity Acknowledgement
I understand that I will be participating in a fishing charter on the Mallacoota lake system operated by Mallacoota Fishing Charters and Tours. I acknowledge that boating and fishing involve inherent risks, including but not limited to slipping, falling, hooks, rods, reels, weather changes, water conditions, and the movement of the vessel.
Assumption of Risk & Release of Liability
I understand that participating in a fishing charter involves some inherent risks. By joining the charter, I voluntarily accept these risks and agree to release Mallacoota Fishing Charters and Tours, Brock York, its crew, employees, agents, and vessel owner from liability for any loss, damage, injury, or death arising from or during the charter, to the fullest extent permitted by law.
Medical Fitness
I confirm that I am medically and physically fit to participate in boating and fishing activities. I will inform the operator of any medical conditions, allergies, injuries, disabilities, or medications that may affect my safety or the safety of others on board.
Alcohol, Drugs & Behaviour
I agree not to consume alcohol or drugs before or during the charter in a manner that impairs my safety or the safety of others. I will follow all instructions given by the skipper, including the wearing of life jackets when directed.
Photography & Video Media Release
I understand that photos and videos may be taken during the charter and may include me, my voice, or my catch. I give Mallacoota Fishing Charters and Tours permission to use this content for promotional purposes, including social media, the website and advertising. If I do not wish to be photographed or filmed, I will let the skipper know before the charter begins.
Agreement
I have read this waiver and media release carefully, understand its contents, and agree to be bound by it. I acknowledge that I am signing this agreement voluntarily and that no oral representations have been made to induce me to sign.
